The diagnosis and the procedure are different facts
Cornell describes canine pyometra as a potentially life-threatening uterine infection requiring prompt treatment. Surgery commonly removes the infected uterus and ovaries. A procedure resembling a spay can therefore have an illness-driven purpose. The veterinarian should document that purpose; an insurance editor should not diagnose it from a bill. Cornell’s cited page concerns dogs, not a treatment protocol for every species.
Read the exception as well as the exclusion
One concrete example is the Pets Best Alabama specimen, form IAIC-PB 10001-ILL, checked October 8, 2026. Section 5.A.2.c has a spay/neuter exclusion with an exception tied to a veterinarian’s recommendation after reproductive-organ damage. Section 5.A.6.i separately addresses breeding and pregnancy-related conditions. This undated specimen does not determine a current owner’s claim; its value is showing why the exact medical cause and exception matter.

Under an invented formula paying 80% after an unused $500 deductible, the $2,800 eligible subtotal yields $1,840. If the remaining $250 is excluded, the owner retains $1,210 of the whole invoice. If those lines qualify too, recompute rather than adding their full price to payment. These are hypothetical accounting inputs, not a quote, medical estimate or guarantee.
